Please note all fees are due upon rendering of services and for remedy costs same day as prescription. An official receipt is provided for you to submit to your insurance company for either Naturopathic or Acupuncture benefit reimbursement. A separate invoice is provided for remedy prescriptions for income tax health credits. Please consult your health care benefits package to help guide your process.
Payment methods include cash payment, cheque or credit card. Both Visa & MasterCard are accepted. Please note there is a 3% service fee for all credit card charges.
Any late payments past due by 30 days are subject to a 2% per month monthly interest accrual fee until balance is paid in full.

Your appointment time is confirmed and reserved just for you!
Please note that a cancellation policy is in effect.
We do require 24 Hrs cancellation notification in order to avoid the cancellation fee for the cost of your scheduled treatment session.

Acupuncture Services can be covered under Naturopathic benefits and scope of practice.

All sales of products and remedies are full and final.
There are no refunds or exchanges on Naturopathic Prescriptions or Service Fees.
Please note that in order to provide safe and effective remedy prescriptions we do not accept any product returns for refund as we cannot ascertain how the product was stored.

Motor Vehicle Accident Claims
For Motor Vehicle Accident Claims, MVAs, FSCO and the auto insurance companies require that all extended health care benefits be utilized in conjunction with or prior to accessing the auto insurance claims benefits. In this case, you will be billed accordingly and some receipts will be issued for reimburseme
Motor Vehicle Accident Claims
For Motor Vehicle Accident Claims, MVAs, FSCO and the auto insurance companies require that all extended health care benefits be utilized in conjunction with or prior to accessing the auto insurance claims benefits. In this case, you will be billed accordingly and some receipts will be issued for reimbursement from extended health care benefits, while the remainder is billed to the auto insurer Each case is different and Insurance Claim Forms are required for approval.
If no extended health care benefits exist, the visits are billed directly to the auto insurer for payment, pending approval from the insurance company for the treatment plan.
If there are no extended health care benefits available, MVA claims may be out of pocket until we receive approval and reimbursement from the auto insurer.
Motor Vehicle Injury Coverage
Dr. Cheryl D' Costa, BPHE, ND & Evolve Naturopathic Vitality Centre are registered and licensed as an authorized practitioner and facility to treat patients injured in motor vehicle accidents through HCAI (Health Claims for Auto Insurance & is licensed with FSCO (Financial Services Commissioner of Ontario) to carry out such duties.
